Yellow Dot Program - Port Jervis Police Department

The Yellow Dot Program is free and designed to provide a quick reference to first responders while providing life saving medical care to a senior citizen or vulnerable individual during a car crash or home emergency. The participating individual would complete a card with basic emergency information including a summary of health conditions, medications, and emergency contact information. If the senior drives a motor vehicle, a yellow dot sticker is placed on the driver’s side rear window and the emergency information card is placed in the glove box. For home emergencies, a yellow dot sticker is placed on the front door and the emergency information card is placed in the freezer inside a plastic zip lock bag. The sticker alerts first responders that the senior is participating in this emergency notification program and signifies where the information card is stored.
